Zeramex P6 Dental Implant Placement | explained by Dr. Shepard Delong

This is a typical workflow for me with the Zeramex P6. The patient did not want an RCT or metal placed in her body. She wanted a high-quality, healthy and life-like tooth replacement. We extracted the tooth and performed ridge preservation with PRF and allograft. After 4 months of healing an implant is selected following standard bone volume requirements using Galaxis from Sirona and a CEREC 2 guide system. This case was narrow buccolingually so a 4.2 diameter implant was chosen and placed. The 0.8-1.8 mm supracrestal placement allows the clinician to place it according to soft tissue thickness and without the need for significant bone profiling. Healing abutments or “gingiformers” can be placed at the time of surgery or utilized in a two-stage approach. Abutment collar heights of 1 or 2mm can be selected, modified by hand and seated or selected for laboratory use. A digital scanning abutment or impression post can be utilized for a lab fabricated restoration. I chose to scan with CEREC Omnicam and then restored with a same-day emax crown. Many options for restorative workflow and I have found great success with both the P6 and XT. I love Zeramex! Patients love it too. I keep saying, “The future is bright, White!”

About the Doctor

Dr. Shepard Delong

Dr. DeLong is a third generation dentist. With experience derived from time spent assisting his father at a young age to performing dentistry in remote locations around the world, he has developed unique perspective, intuition and skills. He has practiced in some of the most advanced dental settings from a technological, medical and holistic standpoint. He is still on staff and works periodically at Pure Health Dentistry on the island of Maui.

Throughout his own health journey, Dr. DeLong has benefitted greatly from the care and services offered by other holistic medical professionals. There is so much current science on wellness and human health that supports this philosophy.  His practice of meditation and yoga, have guided his evolution as a person and dentist.

When setting out to start his own practice, Dr. DeLong had the goals of offering the highest quality dental services in a mindful, holistic, high tech, eco friendly setting. He met Dr. Jeff Williamson during his planning phase and they both saw a great opportunity.  Dr. Williamson has been one of the Northwest’s best holistic dentists for over 30 years and was ready to retire. Dr. DeLong knew the benefits of joining a great mentor and office with all the systems in place to treat patients in a holistic and high quality environment.

In April of 2017 Dr. DeLong purchased Centerpointe Dental and Dr. Williamson retired. The practice has changed its name to Lotus Dental Wellness. The vision of the practice moving forward is to continue providing a high level of care and commitment to holistic, health centered dentistry while implementing new technology and systems to enhance patient care and environmental stewardship.

Practical implantology and implant prosthetics | pip 4 | 2013

The following case report is intended to show the use of the Zeramex Plus implant (Switzerland Dental Point AG Zurich). This implant combines two properties demanded and recommended by science.

What is meant is the use of an industrial, high-quality zirconium dioxide manufactured according to standardized criteria as well as a microstructured and rough implant surface. After several years of experience with one-piece zirconia implants, the author was excited to see the results of a two-part system. The implant insertion does not differ in any way – apart from the mandatory thread cut for every bone quality – from the implantation process in titanium implantology. The two-part system has considerable advantages because the implants can heal unloaded. No special interim prosthetic measures are required. In the prosthetic phase, we can use healing caps as well as screwed down temporary restorations. The Zeraloc connection and abutment bonding let us forget the micro-gap discussions.

The present case describes the treatment of a 53-year-old patient with a Zeramex Plus implant in region 45, after an uncomplicated extraction in September 2012. Due to various intolerances, the patient wished for a zirconia implant. On April 2, 2013, the patient was treated with a ten millimeter implant with a 4.1 millimeter diameter under local anesthesia. The patient received a single shoot dose of penicillin 1.5 grams preoperatively.

Many thanks for the good cooperation to my colleague Dipl. Stom. Dentist Katrin Zenker and master dental technician Dirk Seiring, MSc Dental Technik.

Comment materials

Zirconium Ceramill Zi was used for the crown production, the material shrinks by about 22% after the sintering process. The zirconia body was colored with DD Bio Z color TS immediately before the sintering process. The material of the veneer is zirconia (Heraceram), both masses and glaze pastes are from Heraeus.

Dr. Shelby Nelson of Custom Periodontics & Implantology shares her experience in placing her first ever Ceramic Dental Implant. Watch the video below as she lists down the reasons why she chose to move forward with ZERAMEX XT!

The patient had a Titanium implant with adequate bone and adequate tissue but it produced a gray hue which can’t be masked. It made the patient unhappy so she had it removed and replaced by a Ceramic Dental Implant – ZERAMEX XT. Check out the photos below.

ABOUT ZERAMEX® XT

ZERAMEX®XT has a tapered design allowing optimal primary stability. The internal connection has been specifically engineered to maximize the strength of rigid zirconia and eliminate any lateral forces on zirconia.

The precise fitting components offer variable placement close to bone level for enhanced restorative margin. The heart of the connection is the VICARBO® screw. Comprised of many individual threads of carbon fiber that run vertically, uncut down the length of the screw to durably absorb the tensile forces and give the VICARBO® screw its exceptionally high strength.

Esthetic Rehabilitation with Zirconia Dental Implant on a Complex Pathology Patient

Author: Luis A. Alicea DMD, MSD

The zirconia dental implants have several advantages over titanium implants. The zirconia implant is a high strength ceramic not a metal material, white in color and not visible through the gum tissue, offering a more aesthetically pleasing option for patients. Also, the ceramic is a more biocompatible material that will not corrode over time as other dental implants metal materials. so particles from corrosion are not emitted into the body as a foreign body

Holistic alternative to titanium Implants, ideal for patients with metal allergies. The osseointegration of the zirconia implant is similar as the titanium dental implant. Zirconia dental implant and zirconia dental prosthesis are more resistant to harbor less initially adherent (plaque).

Single tooth restoration with one All-Ceramic Implant Solution in the anterior region

Two-piece ceramic implants from  the latest generation (ZrO2) are trending in the dental market nowadays because of its excellent biological and material technology characteristics as the alternative treatment option compared to titanium implants [1-11]. The handling and the Workflow now correspond to the Demands of modern implantology. The successful use of two-piece Ceramic implants is presented in this clinical case.

Initial clinical and radiological situation: tooth 21 with apical osteolysis.

Case presentation

A 40-year-old patient presented with a tooth 21 that was not worth preserving in our jaw surgery practice before (Fig. 1). The tooth film shows an approx. 7 mm in size apical lightening with widening of the periodontal gap in the upper third of the root (Fig. 1). In view of the clinical situation (high smile line, good oral hygiene) decided we are looking for a two-part ceramic implant Zeramex XT (Dentalpoint AG).

Surgical phase

After extraction of tooth 21, the excochleation of the apical granulation tissue via a semilunar incision (Fig. 2). To avoid failure of the ossification of the ceramic implant and the soft tissue structures (papillae, to protect attached gingiva), we chose a two-stage one Action. A clampless prosthesis made of Valplast served as a temporary restoration. Region 21 was opened after five months (Fig. 3). A two-part ceramic implant Zeramex XT (ø 4.2x L 12 mm) was incorporated (Fig. 4, 5).

The guidelines of implantation in the anterior region and the Adherence to the drilling protocol of the manufacturer was observed. In addition to the transversal, the vertical insertion depth is crucial for a prosthetic success. Because of a special thermal etching process in the collar area the implant can be inserted between 1.6 and 0.6 mm supracrestally, the insertion depth depending on the Gingival height and the existing bone of the neighboring teeth oriented (Fig. 4, 5).

... and from occlusal.
Bone build-up in the vestibular region 21.
Subsequent cover with a membrane.
Clinical condition after exposure
X-ray control after exposure

Since the abutments are applied at least one millimeter from the level of the implant shoulder, the implant should be positioned approximately two to three millimeters subgingivally. The transverse bone structure was mixed from autologous bone chips (retromolar UK), xenogeneic Bone replacement (Bio-Oss) and a membrane (Jason membrane) performed (Fig. 6, 7). After four months, the Exposure using a PEEK gingiva former (Fig. 8, 9).

Individual components: temporary crown, provisional Abutment (PEEK) and temporary screw (PEEK-CW 30).
Components on the model.
Customized abutment made of zirconium oxide with Vicarbo screw from sagittal.
Individually layered, definitive ceramic crown with ceramic structure and ceramic implant from labial and palatal.
Occlusal view on the master model for the definitive one Care.
Crown in place

Prosthetic phase

For optimal contouring of the marginal gingiva Achieving a crown in the highly aesthetic zone made from Gradia, a micro-ceramic composite. Dental film recordings (right angle technology) and the DBSWINSoftware (Dürr-Dental). There is a bone breakdown for six months (mesial bone: 0.5 mm; distal bone: 0.4 mm) and a bone structure for twelve months (mesial bone: 0 mm; distal bone: 0.3 mm) after exposure (Fig. 8, 19, 20). The yellow line on the X-ray images corresponds to the actual implant length (13.6 mm). The red lines correspond to the mesial and distal distances from the top edge of the implant to the first bone contact measured.

Harmonious end result with Gummy Smile.

In our patient, the bone breakdown was in accordance with the literature in the first six months largest. However, the information in the literature in the Contrary to our supply based on one-piece ceramic implants. This phenomenon of bone cultivation is in implantology relatively rare and should be included Evidence-based long-term examinations should be backed up before one general statement. Evaluations are in progress.

Soft tissue situation after six months.
Condition after twelve months with slight improvement 20 the mesial papilla.
X-ray results six months after Exposure.
Results of the X-ray evaluation twelve months after Exposure

Conclusion

Ceramic implants are a good addition to the titanium implants Alternative. Has the advantages of ZrO2-Ceramic itself once you get to know them, you learn to appreciate them and want to no longer do without it.

In addition to the possibility of metal-free restoration she and others For many risk groups, taking into account the indication, a useful addition to the previous treatment spectrum. He believes that the material is relatively new but somewhat different Author user training recommended. This demand for training in handling new materials will also increasingly discussed in the literature.
