Increasing standards of aesthetics and higher incidences of allergies caused by titanium, coupled with greater demands for metal-free dental reconstructions, have resulted in the use of ceramics as a surrogate material in dental implants. Though titanium is still the material of choice in implant manufacturing, zirconia has become an attractive alternative material due to its enhanced aesthetic potential and higher mechanical strength compared to traditional metals.

Zirconia’s superior properties include its high compressive strength, higher tensile strength, and modulus of elasticity compared to any titanium alloy or commercial-grade pure titanium. Tetragonal zirconia polycrystal, specifically around 3-mol% yttrium oxide (yttria)-stabilized zirconia, is mainly the selected ceramic for dental implants. Zirconia’s white, opaque color with low affinity for bacterial plaque and ultra-biocompatibility make it a material of interest in the biomedical industry.

Biomechanically Stable Processing

In its pure state, zirconium dioxide possesses low shear strength and is very brittle, which makes it unsuitable for dental applications. But adding small amounts of aluminum oxide and yttrium oxide increases zirconium’s modulus of elasticity and stabilizes it.

Today, hot isostatic pressing (HIP) subjects monocline zirconia to high pressure, which causes the condensation of particles and results in a tetragonal crystalline lattice structure, where the particles seem to overlap. This significant innovation prevents the propagation of cracks.

With HIP-processed zirconia, any microcrack that develops is quickly stabilized since the tetragonal particles expand into the monocline structure to fill the gap formed. This self-repairing characteristic is also known as the airbag effect.

The modifications of polycrystal “HIP zirconia” material even prevent the material from aging. These key features eliminate breakage. Several experiments have evaluated the biomechanical properties of zirconium implants with significant success.

The 2-Piece Prepable Design

Previously, most zirconia implants were produced as one-piece implants. But these systems had some limitations. It was not always possible to satisfy prosthodontics requirements during the surgical placement, and the angled abutments for correcting misalignment were not available.

Secondary shape corrections with grinding always should be avoided since they could severely degrade zirconia’s fracture strength. Also, one-piece implants are exposed to forces from the tongue and mastication. Ensuring suitable torque, choosing a proper thread design, and using a guarded temporary appliance such as Essix can prevent these problems.

Many ceramic systems are offered as 2-piece implants that can be placed just like titanium implants with good primary stability and left to heal under the gums for 3 to 6 months. These implant systems also exhibit sufficient fracture resistance that can withstand strong bite forces.

Also, to obtain high connection strength between the implant and abutment, a non-metal carbon reinforced screw made of high tech plastic (Iso-elastic properties) is needed to withstand tensile forces.

Two-piece implants with screw-retained abutments are preferable for many reasons. Further research will surely lead to improved biomechanical properties that will make it possible to try novel solutions. Enhanced strength also will enable the innovative design of implants, reconstructions, and the components for connecting them.

Soft-Tissue Integration and Reduced Peri-Implantitis

From a biological perspective, zirconia provides very valuable characteristics. It has a very low affinity for bacterial plaque, a negligible inflammatory infiltrate, and remarkable soft-tissue integration. All of these characteristics can diminish the risk of peri-implant diseases.

It is suggested that the bacterial biofilm accumulates less on zirconia compared to titanium. Hence, it may be assumed that peri-implant soft tissues around zirconia implants will be at lower risk of infection and inflammation. Every implant material has a definite surface free energy. It is observed that zirconia abutments have low surface free energy and low surface wettability, reducing adhesion of bacteria.

Presently, with fair clinical experience, peri-implantitis appears to be less of a problem with zirconia implants than with titanium implnts. Actually, peri-implantitis has not been observed or reported so far, though this needs to be confirmed with longitudinal monitoring.

The authors have been in the business for a long time – and more: They have many years of experience with implant-supported dentures. Both specialists have made ceramic implants their great passion (Fig. 1). Klaus Pettinger is even one of the pioneers in this area. Wolfgang Weisser was able to benefit from his specialist knowledge. Together they plan and discuss the handling and pitfalls of ceramic implants. As a result, the idea arose to describe the procedure of the monolithic abutment step by step in a specialist article.

The patient case The loss of the tooth in region 36 (Fig. 2) had caused the patient some inconvenience: food particles accumulated in this area which the man found very annoying. He came to the practice of Dr. Michael Schneider in Schomdorf with the wish to close this gap. After a detailed diagnosis by the practitioner, it was agreed to insert a cervical implant and to remove all metal restorations from the lower jaw in this treatment phase. The renovation should be done with metal-free reconstructions. The surgical procedure The initial situation shows the opening for displaying the bone – performed with a pilot hole (A) with the Zeradrill Pilot 2.3 mm and then via the EP drill with the Zeradrill W 10 with a 5.5 mm drill in preparation for thread cutting (Fig. 3).

Then the thread cutting protocol followed. With this arrangement, Zeratap wide with 5.5 mm (Fig. Step, everything for inserting the 4 and 5) is prepared after the surgical implant.

With the pick-up (Fig. 6 and 7) the wide implant is 10 mm long, c Diameter 5.5 mm, used, Dr. Micheal Schneider always sets the same bon level, which is why he is also for the XT from Zeramex supracrestal (Fig. 8) decided.

If the retention holder is perfectly aligned, angled standard abutments can later be used in the restoration (Fig. 9). You can clearly see the concave platform – a ten degree cone. After inserting the XT 17510, the wound is closed with the Healing Cap WB 37500 (Fig. 10) – with a Dr. Tailors preferred thin sutures (Fig. 11), which are very comfortable for the patient. After the healing phase, a slightly fenestrated gingiva can be seen over the healing cap (Fig. 12).

The opening is made through the incision for the placement of the gingiva former {Fig. 13). The practitioner removes the healing cap (Fig. 14), the implant is cleaned and replaced with the healing cap WB 37503 (Fig. 15). You can see the situation very well (Fig. 16). It would also be possible to create a custom healing abutment to prepare the space for the later custom abutment.

The second part of the article is about taking an impression and inserting the individual abutment.

Zeramex P6 Dental Implant Placement | explained by Dr. Shepard Delong

 

This is a typical workflow for me with the Zeramex P6. The patient did not want an RCT or metal placed in her body. She wanted a high-quality, healthy and life-like tooth replacement. We extracted the tooth and performed ridge preservation with PRF and allograft. After 4 months of healing an implant is selected following standard bone volume requirements using Galaxis from Sirona and a CEREC 2 guide system. This case was narrow buccolingually so a 4.2 diameter implant was chosen and placed. The 0.8-1.8 mm supracrestal placement allows the clinician to place it according to soft tissue thickness and without the need for significant bone profiling. Healing abutments or “gingiformers” can be placed at the time of surgery or utilized in a two-stage approach. Abutment collar heights of 1 or 2mm can be selected, modified by hand and seated or selected for laboratory use. A digital scanning abutment or impression post can be utilized for a lab fabricated restoration. I chose to scan with CEREC Omnicam and then restored with a same-day emax crown. Many options for restorative workflow and I have found great success with both the P6 and XT. I love Zeramex! Patients love it too. I keep saying, “The future is bright, White!”

Practical implantology and implant prosthetics | pip 4 | 2013

The following case report is intended to show the use of the Zeramex Plus implant (Switzerland Dental Point AG Zurich). This implant combines two properties demanded and recommended by science.

What is meant is the use of an industrial, high-quality zirconium dioxide manufactured according to standardized criteria as well as a microstructured and rough implant surface. After several years of experience with one-piece zirconia implants, the author was excited to see the results of a two-part system. The implant insertion does not differ in any way – apart from the mandatory thread cut for every bone quality – from the implantation process in titanium implantology. The two-part system has considerable advantages because the implants can heal unloaded. No special interim prosthetic measures are required. In the prosthetic phase, we can use healing caps as well as screwed down temporary restorations. The Zeraloc connection and abutment bonding let us forget the micro-gap discussions.

The present case describes the treatment of a 53-year-old patient with a Zeramex Plus implant in region 45, after an uncomplicated extraction in September 2012. Due to various intolerances, the patient wished for a zirconia implant. On April 2, 2013, the patient was treated with a ten millimeter implant with a 4.1 millimeter diameter under local anesthesia. The patient received a single shoot dose of penicillin 1.5 grams preoperatively.

Comment materials

Zirconium Ceramill Zi was used for the crown production, the material shrinks by about 22% after the sintering process. The zirconia body was colored with DD Bio Z color TS immediately before the sintering process. The material of the veneer is zirconia (Heraceram), both masses and glaze pastes are from Heraeus.

About the Doctors

Michael H. Morgan, D.D.S, M.S.

Michael H. Morgan, D.D.S., M.S., received an MS, a certificate in Periodontology, and his DDS from the Ohio State University College of Dentistry.  In private practice since 1994,  Dr. Morgan has earned a reputation for excellence in implant dentistry, and especially complex cases referred to him throughout the region.  He also focuses on periodontal plastic surgery and laser assisted periodontal regenerative procedures (LANAP).

While maintaining private practice, Dr. Morgan was also on staff at the Cleveland Clinic Foundation, one of the largest hospital-based dental facilties in the United States.  He was a faculty member in Case Western Reserve University’s Graduate Periodontology program.  He has participated in dental missions to Peru and continues to travel to the Phillippines to assist Helping Hands Medical Missions.

In addition to authoring publications, Dr. Morgan has leactured at numerous international national, regional, and local programs during his career, including the American College of Prosthodontics and the Pittsburgh Society of Periodontologists. He is a member of the American Academy of Periodontology, the Academy of Osseointegration, the American Dental Association, the Ohio Dental Association, and the Greater Cleveland Dental Society.  In addition, he is a member of the Connecting Science Circle, a group of opinion leaders for ThommenMedical. 

Dr. Matthew Lemke
Matthew J. Lemke, D.D.S, M.S.

Matthew Lemke, D.D.S., M.S., spent three years as a General Dentist before limiting himself to periodontics.  Outside of treating periodontal disease, Dr. Lemke has extensive experience with hard and soft tissue augmentation, implants and peri-implantitis procedures.  With his experience and education, Dr. Lemke aims to provide the highest level of care.

After receiving his B.S. at John Carroll University and serving as an EMT, Dr. Lemke received his D.D.S., certificate in Periodontology and MS all from The Ohio State University College of Dentistry.  While in the Periodontal Program he completed his Master’s research on ridge preservation, CT and micro-CT scan imaging and was able to work on innovative soft tissue augmentation techniques.  Dr. Lemke currently belongs to the American Academy of Periodontology, American Dental Association, the Ohio Dental Association, and the Greater Cleveland Dental Society.  He is also a certified Diplomate of the American Board of Periodontology.  Aside from private practice, he spends time as an adjunct professor for Case Western Reserve University.

About the Doctor

Dr. Henriette Lerner is the founder and Director of HL Dentclinic and Academy in Baden Baden, Germany, which is an academic clitical, teaching and research facility of the Johann Wolfgang Goethe University, in Frankfurt on Main, Germany.

Dr. Lerner is the Past President of the Digital Dentistry Society International (ODS).

In 1990, Dr. Lemer was granted the degree of Doctor Medie (DMD degree from the Faculty of Dental Medicine, in Temeschburg, Romania and then completed har post graduate education in Oral Surgery, in Bucharest Romania. Later, Dr. Lerner’s education In Implant Dentistry was completed in Germany and at Boston University.

Among her other certifications, she is a Board Member & Expert for the DGOI The German Society of Oral Implantology.and is an Icoi Diplomate Dr. Lerner also an Editorial Advisor for two scientific journals (Practical Implantology and DGO!

Esthetic Rehabilitation with Zirconia Dental Implant on a Complex Pathology Patient

The zirconia dental implants have several advantages over titanium implants. The zirconia implant is a high strength ceramic not a metal material, white in color and not visible through the gum tissue, offering a more aesthetically pleasing option for patients. Also, the ceramic is a more biocompatible material that will not corrode over time as other dental implants metal materials. so particles from corrosion are not emitted into the body as a foreign body

Holistic alternative to titanium Implants, ideal for patients with metal allergies. The osseointegration of the zirconia implant is similar as the titanium dental implant. Zirconia dental implant and zirconia dental prosthesis are more resistant to harbor less initially adherent (plaque).

About the Author

Luis A. Alicea DMD, MSD

Dr. Luis A. Alicea was born and raised in San Juan, Puerto Rico. He pursued studies at the University of Puerto Rico, School of Dentistry. He then completed his Masters Degree, and Prosthodontic residency at that campus, where he focused his training in the placement, and restoration of Dental Implants. He has taught as an Associate Professor in the Research, and Restorative Departments at the University of Puerto Rico, and continues to hold privileges as a professor Ad Honorem in that institution. Dr. Alicea also completed additional training towards certification as an IV Sedation provider, at the University of Alabama, Birmingham.

Dr. Alicea is a member of the Peer Review Committee for “The International Journal of Oral & Maxillofacial Implants”, and regularly reviews articles prior to publication for this Journal. He is a also member of the American Dental AssociationAmerican Academy of Cosmetic DentistryAmerican College of ProsthodonticsFlorida Prosthodontic AssociationAmerican Academy of Implant DentistryAcademy of OsseointegrationAmerican Dental Society of AnesthesiologyWest Coast District Dental AssociationHillsborough County Dental Association, and also participates in the Seattle Study Club.
