The portfolio of the 100 percent metal-free and reversibly screwable Zeramex XT ceramic implant system is now complete. After the launch of the new competence center Zeramex Digital Solutions, which serves the demand for individual abutments and one piece, monolithic crowns with implant connection, Firrna Dentalpoint is launching further innovations on the market. The portfolio of the Zeramex XT implant system now also includes the anterior teeth in the lower jaw and lateral incisors in the upper jaw Implants with an endosseous diameter of 3.5 mm (Fig. 1a). The new small-base implants (SB) (Fig. 1) are available in lengths of 8, 10 and 12 mm. In addition to the familiar regular and wide-base platforms with endosseous diameters of 4.2 mm for the RB implant and 5.5 mm for the WB implant, the range of Zeramex XT implants is now complete (Figs. 2 to 5).

In addition to the SB implants, the system has also been expanded with new abutments for removable dentures. The Zeramex Docklocs abutments are the first 100

Patterns of COVID-19 Mortality and Vitamin D: An Indonesian Study

Key Findings:

  • Majority of the COVID-19 cases with insufficient and deficient Vitamin D status died.
  • The odds of death was higher in older and male cases with pre-existing condition and below normal Vitamin D levels.
  • When controlling for age, sex, and comorbidity, Vitamin D status is strongly associated with COVID-19 mortality.
  • Randomized controlled trials are warranted to investigate the role of vitamin D supplementation on COVID-19 outcomes and to establish the underlying mechanisms.

ABSTRACT

This is a retrospective cohort study which included two cohorts (active and expired) of 780 cases with laboratory-confirmed infection of SARS-CoV-2 in Indonesia. Age, sex, co-morbidity, Vitamin D status, and disease outcome (mortality) were extracted from electronic medical records.

The aim was to determine patterns of mortality and associated factors, with a special focus on Vitamin D status. Results revealed that majority of the death cases were male and older and had pre-existing condition and below normal Vitamin D serum level.

Univariate analysis revealed that older and male cases with pre-existing condition and below normal Vitamin D levels were associated with increasing odds of death. When controlling for age, sex, and comorbidity, Vitamin D status is strongly associated with COVID-19 mortality outcome of cases.

We are excited to announce that ZERAMEX®USA has been selected to become the front cover of the international C.E. magazine of oral implantology, Implant Tribune for their April 2020 issue!

Featured on the front cover is our latest Ceramic Dental Implant, ZERAMEX®XT. A two piece, screw-retained system that functions surgically and restorative like current titanium implants. The current product offer has restorative flexibility with multiple abutment choices and will soon have scan bodies and a complete digital workflow with fully customized prosthetics. It is tapered in design, has an internal connection and the unique VICARBO® screw creates a seal against bacteria and micromovement. Modern ceramic implants now offer the same ease of use as current titanium implants.

Also featured in the April 2020 issue is the case presentation of Dr. med. dent. Jens Tartsch, DMD featuring ZERAMEX® XT.

Dr. Jens Tartsch

This case involved a simple placement of an implant in a site with adequate bone in an overall healthy patient. The case is significant in that it was the first case ever done worldwide using the ZERAMEX XT Ceramic Implant System.

It shows a ZERAMEX XT 4.2 x 10mm implant placed in a healed extraction and grafted site. The implant placement was done 4 months post tooth extraction and grafting. The implant was restored using the VICARBO Carbon Fiber screw and zirconia abutment. At one year follow up, the implant shows no peri-implant inflammation. No movement or bone loss can be observed.

This case was done in Zurich Switzerland by Dr. Jens Tartsch. Dr. Tartsch practices in Zurich Switzerland and is a founding father of the European Society of Ceramic Implants. ESCI is a connection between industry leaders and clinical users of ceramic implants working together to create the designs, and technologies for proven future success.

The SmartPeg is used together with with an ISQ measurement instrument – either the Osstell IDx or Osstell Beacon. Both instruments measure the resonance frequency of a SmartPeg, when it is attached to an implant. The SmartPeg is attached to the implant or abutment when a measurement is made. It is easy to mount and requires minimal space thanks to its small size. To avoid damage to the implant, the SmartPeg material is quite soft, which limits the lifetime of the threads.

Abstract: Implant stability is a key consideration when determining the point in time a dental implant should be loaded. Often a change in implant stability is observed as healing progresses from initial mechanical stability to biologic stability, ie, osseointegration, during the bone modeling/remodeling process. This change in stability can be objectively measured using resonance frequency analysis (RFA). Because bone healing varies from patient to patient, the timing of this sequence may be unpredictable; in a small subset of patients the drop in implant stability can be quite precipitous and profound. Such a drop is referred to as a “bungee dip.” If the implant is loaded too prematurely, it may fail due to inadequate healing. This article presents two case reports that demonstrate the use of RFA and its values, known as implant stability quotients, to monitor implant stability during healing. RFA is a form of personalized care that provides objective evidence that can guide the clinician in establishing safe and successful loading protocols.

Case Preview:

Case 1: Mandibular First Molar

A 63-year-old female patient in good general health was referred for evaluation and placement of an implant at the site of a failing mandibular left first molar. The tooth previously had undergone endodontic treatment more than a decade prior but had recently become symptomatic. Clinical and radiographic examination of the area revealed no mobility of the teeth, probing depths ranging from 1 mm to 3 mm (Figure 1), and a large radiolucent lesion at the mesial root of the mandibular left first molar (Figure 2). Based on these considerations and consultation with an endodontist, the prognosis for this tooth was questionable to hopeless.

Case 2: Maxillary Second Molar

A 54-year-old female patient in good general health was referred for evaluation and placement of an implant at the site of a missing maxillary left second molar (Figure 11). Pretreatment assessment determined that adequate bone was available in both the buccal-palatal and vertical dimensions for implant placement without the need for any additional procedures.

Following reflection of a full-thickness flap, the implant osteotomy was prepared and a 4.5 mm diameter x 9 mm length tapered dental implant with an electrowetted surface (ProActive®) was placed into type 2 bone. The final seating torque of 35 Ncm and ISQ values of 71 and 70 (Osstell IDx) in the buccal-palatal and mesial-distal directions, respectively, confirmed adequate primary stability before flap closure (Figure 12). A PEEK healing abutment was placed (Figure 13) prior to flap suturing, and the implant was allowed to heal transgingivally.

Professor Sandhaus’ whole scientific career was based on teaching, research and the perfection of new techniques and efficient instruments. He dedicated his life to research and especially to innovation in the field of oral implantology. He was widely recognized as the founder of modern metal free implantology using zirconia bioceramics.

He was the Director of the Forum Sévelin and President of honour and founder of the International Society for Oral Rehabilitation. He was awarded in 1959 with the diploma of medicine and dentistry, by the University of Düsseldorf, followed a specialization in dentistry and maxilla-facial surgery at the Paris Ⅵ University, under the guidance of Professor Deschaume.

His works were based on the execution of implants which do not release oxides in the osseous tissues, as metallic implants do. The first ceramic implant of Prof. Sandhaus came to light in 1960: It was the CBS implant (Crystalline Bone Screw) made of aluminium oxide. A career based on teaching, research and on the achievement of high-quality techniques and instrumentation (more than 75 original inventions), this was the scientific evolution of Professor Sandhaus. He dedicated 13 years of his life to teaching at the Fortbildung Institute of Karlsruhe in Germany, and from 1985 was a professor at the Pierre and Marie Curie University, Paris Ⅵ, the Dentistry and Maxilla-Facial Institute. Let’s not forget the many conferences given throughout the world, from Japan to the United States, passing through all the European countries, accompanied every time by practical and surgical demonstrations of bioceramic implants.

Below is an interview of Dr. Sammy Noumbissi, Founder and President IAOCI, with Prof. Dr. Sami Sandhaus at the 6th World Ceramic Implantology Congress last February 2017.

The legacy of Professor Sandhaus will always be alive, and he will always be remembered as a father of modern ceramic implantology and an outstanding pioneer in the dental industry.
